It may NOT be emailed.<\/li><li>Filling out the Absence form <span style=\"text-decoration: underline\">does not<\/span> guarantee an excused absence.<\/li><li>Entries to absence forms will be pulled at 1pm each day. <strong>Please allow 48-72 hours for change to occur in HAC.<\/strong><\/li><li><span style=\"text-decoration: underline\">Only<\/span> a parent or a legal guardian may fill out absence forms unless you are submitting a doctor\u2019s note.<\/li><li><strong>Any absence over 15 days<span style=\"font-size: 16px;font-style: normal\"> req<\/span>uires official documentation to be excused.<\/strong> (Medical notes OR statement from court)<\/li><li><span style=\"text-decoration: underline\">Only<\/span> a physician\u2019s office is authorized to fax a note for the student to be excused. Please be available and include the best phone number to reach you on the form. Students will NOT be issued a pass without speaking to a parent or guardian.<\/li><li>To excuse an early release absence, required documentation must be uploaded using the Excused Absence Form or turned in to the front office within 2 days of returning to receive a tardy slip.\u00a0<em><em>We will continue to accept paper-based notes for tardies, or you may complete the online Excused Absence form and upload documentation.<\/em><\/em><p>\u00a0<\/p><\/li><li>When presenting a doctor\u2019s note or a parent note, please make sure the following is reflected on the note:<\/li><\/ul><ul><li style=\"list-style-type: none\"><ul><li>Full Student Name<\/li><li>Student ID #<\/li><li>Date tardy<\/li><li>Parent or Physician signature<\/li><\/ul><\/li><\/ul><p>*Note: If your student is tardy due to a medical appointment, a note from the physician is necessary for the tardy to be excused.<\/p><p>Examples of excused tardies:<\/p><ul><li>Illness<\/li><li>Physician\u2019s arriving tardy to school, students will need to go to the Front Office to receive a tardy slip. <em>We will continue to accept paper-based notes for tardies.<\/em><\/p><p>When presenting a doctor\u2019s note or a parent note, please make sure the following is reflected on the note:<\/p><ul><li>Full Student Name<\/li><li>Student ID #<\/li><li>Date tardy<\/li><li>Parent or Physician
